The usual reasons are a file that is bigger than the limit, the wrong width and height, a photo saved in a format the form does not accept, or a signature that is hard to read. Tips for a clear signature
Sign with a dark pen on plain white paper and photograph it in daylight so there are no shadows. Crop close to the signature, because empty space makes the signature smaller and harder to read after resizing. Do not use a stylus signature from a phone screen unless the form allows it.
